When you call for an appointment, we can usually see you on the same day.
There’s no charge for us to take a look at your oral health and determine how we can help you. After this free consultation, we’ll get your insurance information to verify coverage. Your first visit takes about an hour and does not include a full exam or x-rays.
The required paperwork for this visit including a medical history and a quick questionnaire can be found below. Please fill and out bring the completed forms with you.
